Press start. The transcript arrives on the left, the checklist fills as you cover each element, and the finished note assembles on the right. Then it tells you what you left out.
Sample dictation. The patient, the injury and the note are invented. Your microphone is never accessed, no audio is recorded, and nothing on this page contacts a server. The real app transcribes on the device itself.
Right shoulder pain since Aug 4, onset while lifting a 45 lb carton overhead at work. Rated 6 of 10, worse with overhead reach. No numbness or tingling in the arm.
Tenderness over the supraspinatus insertion. Abduction limited to 90 degrees. Empty can test positive. Neurovascularly intact distally.
Right supraspinatus tendinopathy, work related.
NSAIDs, home exercise programme, physical therapy twice weekly for four weeks.
The part that is not transcription
A transcript is a record of a conversation. A note is a document that has to hold up to an adjuster, a payer and, occasionally, a lawyer. The gap between the two is the two lines you forgot, found while the patient is still in the room rather than three weeks later.

HIPAA Dictate is on the App Store for $49 a month. Or book an assessment and we will look at where the documentation time actually goes.
